We believe in better leveraging existing technology, proactively engaging at-risk patients, and creating a delivery model that is capable of initiating all asthma and COPD patients within a defined community. We start by identifying asthma and COPD patients who are undermanaged and whose disease is not controlled through claims data analysis. Our community-based approach means we go beyond the basic software program that indicates an individual is a likely program candidate and identify appropriate patients through relationships with employers, community physicians and simply through our involvement within a community.

To inspire respiratory health and management, we believe there is no one-way to contact a population of people. Once a list of program candidates has been identified, Spiras begins a campaign to engage these individuals that may include anything from phone calls and text messages to knocking on doors.

After being identified and engaged, we don’t suggest a patient go see a provider. We send providers to patients. A specialized clinician will visit the patient in their home and provide a focused history and physical including a lifestyle assessment and conduct other diagnostics to determine a proper diagnosis. Next, an individualized treatment plan is developed which may include lifestyle adjustments, medications and other treatment services. Substantial time is spent educating the patient on their treatment plan to make sure they know what to do and when to do it.

Once the initial assessment is complete, a series of follow-up visits are scheduled where we perform further diagnostics to ensure the treatment plan is working, check compliance and provide further education.

Spiras monitors both lung function and patient compliance on a continual basis. This allows us the ability to be responsive to an individual patient’s day-to-day needs. It also allows the chance to intervene before an adverse event leads to an emergency room visit, or worse. Understanding our communities, Spiras will utilize both high tech, digital solutions and more simple, analog solutions for patient monitoring.
